This form gives you the possibility to make a request for:

• A new or modification of a GSM subscription.
• Exceptional rights in the use of the GSM phone, or a change in them.
• A request for the purchase of new GSM phone can be done in combination with a request of a new subscription.
• A request for the purchase of a new GSM phone only, without a "new subscription" request, can be done using the "Material Request form".
• Reactivating a subscription - use this option:
  • When your phone has been lost or stolen (see note).
  • To reactivate a subscription which was deactivated.

Note: In case of a lost or stolen GSM, you will have to fill in a separate request for a new phone. You will have to follow the procedure for lost/stolen phone. Comments and explanations can be added in the Additional information comment box on the bottom of the form.

The user will be notified by Labo Telecom, when the request has been treated by E-mail, Fax or internal mail, depending on the person's choice.
